What to Expect for Your First Appointment

Please arrive at the office at least 15 to 30 minutes before your appointment to allow time for the registration process. If you do experience an unavoidable delay, please notify the office so that we can determine if we can alter the schedule to accommodate your appointment.

Please bring the following items to your first appointment:

  1. New Patient Paperwork: You will be asked to complete a number of patient forms.If time allows, they may be mailed to you so you can complete them prior to your appointment. If this is not possible you will need to arrive at the office one hour prior to your appointment time to complete these forms.
  2. Insurance and I.D. cards: Be sure you have your insurance card and driver's license (or other government issued photo I.D.) with you.
  3. Payment: Your insurance co-payment or any out-of-pocket fees due at your appointment.
  4. Medications: Please bring a complete, up-to-date list of all medications and their dosages, or bring the bottles and packages with you.
  5. Any additional medical records, X-rays/MRI's: related to your pain evaluation or treatment.
  6. List of questions and concerns: It may be helpful to bring a written list of questions on your first visit to review with the provider.

When you arrive at the office for your appointment the Receptionist will greet you and will:

  1. Verify some basic information (address, date of birth, phone number, Social Security number, etc.)
  2. Make copies of your health insurance card(s) and photo I.D. Card.
  3. Collect your insurance co-payment or other fees.
  4. Ask you to sign additional authorization forms.
  5. Inform the staff when you have completed the registration process.

Your Initial Evaluation Appointment will take about 2 to 3 hours at the clinic.

Once you have completed the registration process the medical assistant will check your blood pressure and other vital signs and then escort you to the exam room. The medical assistant will also verify your medical information and prepare you for your appointment with the medical provider.

You will have a complete evaluation with one of the medical providers. You may be seen by more than one medical provider during your appointment.

Additional tests and evaluations may be ordered. These may include laboratory tests, x-rays, sleep studies, a physical therapy evaluation, or other diagnostic procedures. We will give you a referral for these tests and evaluations before you leave the clinic.

You may be given one or more prescriptions for medication. You will be given additional information about these prescriptions, including a medication agreement and the clinic prescription and refill policy.

You may be scheduled for an injection or other procedure. Many insurance companies require pre-authorization for any procedure. As a service to you, we verify and pre-authorize all procedures for your convenience. For this reason, it is essential that you provide us with your insurance information and keep your information updated.

Before leaving the clinic you can make your follow-up appointment with the provider's medical assistant. It is usually best to make your appointment at this time since the appointment schedule fills up quickly.
