New Patient Information
Welcome to Summit Pain Management! We are a consultation clinic specializing in the diagnosis and treatment of chronic pain. We are dedicated to providing exceptional and compassionate care for patients with pain. We will work with you and your primary care provider or referring physician to achieve these goals.
For this reason, all patients must have a primary care provider. Our clinic specialists work as a team which includes the pain medicine physicians, physician assistants, nurse practitioners, and the clinical support staff.
During the course of your treatment you may be seen by any of our providers.
Please complete the enclosed forms and bring them, and the items listed below, to your first appointment.
You will be given additional forms to sign when you arrive at the clinic.

Office Hours
Please call during regular office hours.
Emergency calls to the clinic after hours will be given a number to reach the on call provider.
Monday, Tuesday and Thursday: 8:15 am - 5:00 pm
Wednesday: 9:00 am - 5:00 pm
Friday: Closed
Weekends and Holidays: Closed
Appointments and Payments
We schedule appointments during regular office hours.
Please bring to every appointment:

Your co-payment or payment for uncovered charges.
Payment is due at the time of service. We accept cash, checks and credit cards. If you do not have your co-payment at the time of service, your appointment may be rescheduled. - Your insurance card, so your insurance company may be billed. Most insurance plans are accepted.
- A photo ID-government issued, preferably your driver's license.
- Medication list-up to date list of all your medications.
To accommodate all patients we have an Appointment Cancellation Policy
Please contact the office at least 24 hours in advance if you need to cancel any office or procedure appointments. Failure to give advance notice of appointment cancellation may result in fees may being applied to your account to be paid before you may reschedule an appointment.
Prescriptions
- Prescriptions & refills are available during regular office hours only.
- Patients receiving controlled substance medication prescriptions are asked to sign a medication agreement and to follow our prescription policy. These prescriptions are closely monitored.
Directions
We are conveniently located immediately off the I-15 freeway at the 5300 South exit.
Go one block east to 320 West, then go north on 320 West to the Atrium Building at 5250 South.
Handicapped parking and wheelchair access are available.
