First Time Patients
We want your first visit to our clinic to be a pleasant, trouble-free experience. Here are a few easy steps you can follow to make sure things go smoothly:
- Make sure you have received a referral from your primary care provider or treating physician.
- Check with your health insurance provider to confirm that your insurance will cover your appointment with our clinic provider(s).
- Read our New Patient Information page before your appointment.
- Take a few minutes to become familiar with our medical providers and staff.
- Read about what to expect on your first visit.
- Get driving directions to our office.
Things to Bring to Your First Appointment
- When you visit Summit Pain Management for the first time, please remember to bring:
- New patient paperwork.
You should receive this in the mail a few days before your appointment. - Insurance card.
We accept most insurance carriers, and we will need your card to bill the insurance company. - ID card.
Please remember to bring your driver’s license or another government issued photo ID. - Payment.
You will need to pay your insurance co-payment or other out-of-pocket expenses at the time of your appointment. - Medications list.
Please bring a complete, up-to-date list of all the medications you are taking and their dosages. - Additional medical records.
Please bring any medical records, X-rays, or MRIs that relate to your pain evaluation or treatment. - List of questions or concerns
We want to make sure you have the opportunity to discuss any issues with your clinic provider.
