Q: What are your hours?
A: Hours: Monday – Wednesday: 8:30am to 4:30pm. (Closed for lunch from 12:30-1:30pm)
Thursday: 9:00am to 4:30pm (Closed for lunch from 12:30-1:30pm)
Friday: 8:30am to 12:00pm

Q: How can I reach a medical provider after hours?
A: You can reach our answering service at 801-408-6184. We always have a healthcare provider on call to assist patients with urgent needs.

Q: Why should I choose Summit Pain Management?
A: Summit Pain Management has been providing exceptional care for 7 years. We have a staff of compassionate healthcare providers who work in cooperation with you to reduce pain and improve function. We use multiple strategies and treatments to help manage your pain. Most procedures (spinal injections) are done right in our office, using fluoroscopic guidance in our state of the art procedure room. Because you aren’t required to go to a hospital or surgical center for procedures, the cost to you and your health insurance carrier is much lower than that of some other pain management providers in the area. With deductibles and co-insurance on the rise each year, this is one sure way to reduce the impact healthcare has on your pocket-book.

Q: Do I need a referral to be seen?
A: We don’t require a referral, but we will need to review medical records from your past healthcare providers before you are seen. Please call our Patient Coordinator at 801-747-3817 for more information on how to become a patient.

Q: What insurances do you accept?
A: We accept most major insurances, too many to list here in FAQ. We are currently not accepting Medicaid (traditional, Select Access, Molina, Healthy U). We accept all Select Health plans except Select Value. We are not accepting new self-pay patients at this time. Q: How do I request a prescription renewal in between appointments?
A: Please call the Prescription Renewal Line at 801-747-1190.

Q: Can you call my prescriptions in to the pharmacy?
A: Schedule II controlled substance medications require a written prescription and cannot be called in to the pharmacy. Most pain medications fall into the Schedule II category. Hydrocodone is the exception that can be called into the pharmacy. For more information on the Controlled Substances Act go to http://en.wikipedia.org/wiki/Controlled_Substances_Act

Q: Can someone else pick up my written prescriptions for me?
A: You may have someone else pick up your prescription. You’ll need to call us and let us know who you’ll be sending, and they’ll need to present government issued photo identification to pick up the written prescriptions for you.

Q: Can you mail my prescriptions?
A: If you live in Salt Lake County, or southern Davis County we will not mail your prescriptions. If you live outside of those boundaries, your healthcare provider may approve for your prescriptions to be mailed to you.

Q: Do you prescribe Suboxone?
A: No we do not. Suboxone is used for the treatment of opioid dependence. We do not treat opioid dependence/addiction at Summit Pain Management.
